Please note: Receipt of your payment ASAP will secure your seats at the wine dinner. Signing this waiver or an email request will not hold your place. Also note that we can’t guarantee refunds for cancellations received after the deadline. Thank you.
By submitting this RSVP, and in consideration of me and my guests’ participation in the wine tasting and dinner (the “Event”) hosted by Les Amis du Vin (“LADV”) and the restaurant or venue at which the Event is held (the “Venue”), I acknowledge that there are risks (both known and unknown) associated with such participation, including risks inherent in the consumption of alcohol, which risks may include but are not limited to injury, property damage and even loss of life. I knowingly and voluntarily accept and assume any and all risks and consequences associated with such participation. I hereby release and discharge, and agree to indemnify and hold harmless, LADV, the Venue and any officer or employee thereof and any other individuals or entities that may be affiliated with or act on behalf of LADV or the Venue from and against any liability whatsoever, including costs and expenses such as attorney fees, incurred as a result of or in any manner relating to my participation in the Event, including but not limited to any acts or omissions by LADV, the Venue or any other Indemnified Party in connection therewith. I shall comply with all applicable laws, rules and regulations in connection with our participation at the Event. I am over the age of 21.
